Please review the following terms and conditions:

I understand that certain risks are associated with my dog’s or my own participation in activities at Found Chicago Boarding & Training Center, or by being on the premises. These risks include, but are not limited to, physical injury, death, property damage, and other potential hazards.

I hereby release and forever discharge Found Chicago, its agents, volunteers, and their successors and assigns, from any and all liability, claims, demands, actions, or causes of action, damages, and attorneys’ fees that I may suffer, as a result of or arising from my participation or my dog’s participation in any and all activities at Found Chicago Boarding & Training Center, or my presence or my dog’s presence at Found Chicago. This release excludes claims, damages, or liabilities arising from the gross negligence of Found Chicago Boarding & Training Center or its staff, trainers, or volunteers.

If Found Chicago Boarding & Training Center is adjudged by a court of law to be grossly negligent, it shall be liable only for actual damages and not for punitive, special, or consequential damages, nor for damages for pain and suffering, infliction of emotional distress, or similar damage claims.

I agree to disclose any allergies my dog may have to Found Chicago Boarding & Training Center. I further agree to disclose any special dietary needs or medications my dog may require during activities at Found Chicago Boarding & Training Center.

If my dog becomes ill or is injured while participating in activities at Found Chicago Boarding & Training Center, Found Chicago Boarding & Training Center will make every reasonable effort to reach me using the contact information I have provided. However, if Found Chicago Boarding & Training Center cannot reach me, I consent to Found Chicago Boarding & Training Center seeking appropriate veterinary care. I accept responsibility for any and all associated expenses. Found Chicago Boarding & Training Center will not cover any portion of veterinary expenses related to seeking medical care for my dog.

I authorize the use of my dog’s visual image(s) and statements in Found Chicago Boarding & Training Center newsletters, posters, and other materials.

I agree to pay the amount requested for my dog’s participation in activities at Found Chicago Boarding & Training Center. I also agree to pay for any additional services requested, such as grooming or drop-off and pick-up services. A valid credit card must be kept on file at all times so that Found Chicago can cover medical emergencies if necessary.

By clicking this digital box, I acknowledge that if my dog or I am injured, or if property is damaged during my dog’s participation in activities at Found Chicago Boarding & Training Center, I may be found by a court of law to have waived my right to maintain a lawsuit against Found Chicago Boarding & Training Center based on any claim from which I have released it in this document. I confirm that I have had sufficient opportunity to read this entire waiver, that I fully understand it, and that I agree to be legally bound by its terms.
